ResMed's $5.8B Guide Looks Easy-The Real Test Is Whether Cash Flow Stays Strong Enough to Fund $1.85B in Returns

ResMed's new outlook rests on core growth, not a clean break from the Astral issue
ResMed's new outlook is not about asking investors to brush aside the Astral ventilator issue. The real question is whether the company's stronger outlook is coming from a healthier core business or mainly from portfolio cleanup and accounting normalization. That makes this a buy-and-monitor story rather than a set-it-and-forget-it one.
Management gave investors a clear scorecard. ResMedRMD-- finished fiscal 2026 with 9% revenue growth to $1.5 billion, non-GAAP EPS of $2.95, up 16%, gross-margin expansion of 290 basis points, and $1.6 billion in free cash flow. Against that backdrop, its first-ever annual guidance calls for 5-7% core constant-currency revenue growth, 12-14% core EPS growth, and more than $1.85 billion in shareholder returns.
That guidance is notable, but it still has to absorb the Astral impact. Management has flagged an estimated $75 million in revenue and $0.15 EPS headwind from suspending new Astral sales, as well as a $42 million field-safety provision. Bulls can view those as contained, patient-first hits that clear the deck. Skeptics will argue they show how much of the cleaner outlook depends on stripping out noise rather than accelerating the core engine.
The key test is straightforward: does operating cash flow remain strong enough to support both the business and the return program if core growth holds? If it does, the guidance looks credible. If not, investors should treat it as a working hypothesis rather than a promise.
Sleep-apnea demand is broadening beyond a single product cycle
One reassuring point is that the guidance does not rely on a single-product narrative.
Wearables and GLP-1s are helping expand the patient funnel
ResMed is not asking for market share in a flat sleep-apnea market. Management said wearables and GLP-1s continue to boost sleep apnea awareness and treatment initiation. That does not guarantee immediate revenue, but it does suggest a broader demand tailwind than simply leaning on one product launch.

The more direct proof is in the reported mix. Full-year results were supported by double-digit mask growth and the ongoing global rollout of the AirSense 11 platform. That combination matters because masks create recurring demand, while a current device platform gives ResMed a live selling cycle across markets. It is a more credible base for 5-7% core revenue growth than assuming every segment suddenly improves at once.
Portfolio changes matter, but they support the core logic rather than replace it
Noctrix expands the sleep-disorder range through the same channels
The Noctrix acquisition fits ResMed's existing business model. It addresses restless legs syndrome, another sleep-related condition, and management has described it as a way to extend the company's reach through the same HME/DME channels it already leads. Whether that becomes a major near-term revenue contributor is still open, but the strategic fit is clear.
The MatrixCare sale should make the business easier to evaluate
The planned MatrixCare exit also changes the picture. If sold, it should make the remaining company cleaner and easier to value because MatrixCare is outside ResMed's core sleep-and-breathing business. That helps explain why management presents core growth metrics alongside the return plan.
What investors should watch next
The bullish case works if three things happen together: - core revenue growth stays in or above the 5-7% range, - the Astral-related headwind remains contained, and - cash generation stays strong enough to back the company's return program.
If those pieces hold, ResMed's guidance is less about a flashy year and more about a business that is getting cleaner, broader, and more self-funded. If any of those pieces weaken, the story shifts from execution to caution.
