Novo's ZEUS Fail: A 10% Hit, a 3-for-1 Cardio Bet, and the Inflammation Trade Unraveling


ZEUS separated biomarker activity from clinical benefit
In a trial of 6,376 participants, ziltivekimab showed the biological effect it was meant to produce, with expected reductions in free IL-6 and hsCRP. But in people with ASCVD, CKD, and inflammation, that did not translate into fewer major adverse cardiovascular events. The trial reported a hazard ratio of 0.99 with a 95% confidence interval of 0.88 to 1.11, which is a null result at the endpoint that mattered.
That helps explain the market reaction. NovoNVO-- shares fell as much as 10% Friday after the company said ZEUS did not meet its goal of reducing cardiovascular events. The trial population limits how broadly investors can generalize the result
ZEUS was conducted in a high-risk group: participants had a mean age of 69.5 years, 65.7% had diabetes, 41.3% had heart failure, and the baseline mean eGFR was 44.5 mL/min/1.73 m2. That makes the negative result important for cardiovascular inflammation as a broader thesis, but it does not completely close the door on the idea that IL-6 inhibition could still matter in other patient groups or trial designs.
ZEUS adds pressure to Novo's broader cardiovascular program
The failure also lands against a more expensive and less certain backdrop. According to BioPharma Dive, Novo gained control of ziltivekimab through a 2020 acquisition of Corvidia Therapeutics, for which it paid $725 million up front and promised up to $2.1 billion in total payouts. The company said it will take a non-cash impairment charge in the third quarter, though it said the result should not change its 2026 operating profit forecast.
That is why ZEUS matters beyond one asset. It calls fresh attention to the rest of ziltivekimab's late-stage program and to Novo's broader cardiovascular strategy at a moment when investors are less willing to pay for unproven biological bets.
