Elevance Health and NACHC Collaborate to Integrate Nutrition in Primary Care

Elevance Health is integrating Food as Medicine into its care model through a partnership with the National Association of Community Health Centers (NACHC). The program aims to reduce food and nutrition insecurities while enhancing chronic disease management and preventive health outcomes. Primary care teams will be trained to screen and connect Medicaid patients with personalized nutrition support. Elevance Health Foundation will also support NACHC with a six-month Food as Medicine grant.
