The CooperCompanies (COO) shares fell 15.33% in pre-market trading after the medical device maker reported a fiscal third-quarter revenue miss and cut full-year guidance below analyst estimates.

The company posted Q3 revenue of $1.066 billion, roughly 1% organic growth year-over-year but about $30 million below the $1.10 billion consensus. Non-GAAP EPS of $1.15 topped the $1.12 estimate by $0.03, aided by a $307.2 million discrete tax benefit and $0.03 in tariff refunds. GAAP EPS came in at $2.24.

The bigger issue for investors was guidance. Full-year adjusted EPS was lowered to $4.51-$4.55, versus the $4.63 consensus. Fourth-quarter EPS guidance of $1.05-$1.09 fell sharply short of the $1.19 expected, while Q4 revenue guidance of $1.057-$1.080 billion missed the $1.11 billion estimate. Full-year revenue guidance of $4.229-$4.252 billion also came in below the $4.31 billion consensus.

What Did CooperCompanies Report?

Management attributed the revenue shortfall and weak outlook to proactive U.S. channel inventory reductions at its CooperVision contact lens unit, which generated flat $717 million in revenue. The inventory draw, described by President and CEO Al White as a planned move, offset healthy mid-single-digit consumer consumption growth.

CooperSurgical, the fertility and surgical instruments segment, delivered 3% organic growth to $349 million, with fertility revenue up 5%. The MyDay contact lens franchise showed double-digit growth, and MiSight posted 20% organic expansion.

Gross margin contracted 60 basis points to 66.7% on higher costs and foreign-exchange headwinds, though operating margin expanded 30 basis points to 26.3% on productivity gains. Free cash flow reached a quarterly record of $273 million, up 66% year-over-year. The company repurchased $339 million in shares during the quarter and increased its share-repurchase authorization from $2 billion to $3 billion.

Why Did Investors React Negatively?

Despite the EPS beat, the revenue miss and sharply lowered guidance sent COOCOO-- lower. For a high-volume distributor, top-line growth is the market's primary health indicator, and the guidance cut signals that the inventory headwind will persist into Q4.

The Q4 EPS miss of roughly $0.10 below consensus is particularly telling. Management indicated that margins will face pressure from strategic investments, foreign exchange, and lower tariff refunds in the coming quarter, suggesting near-term profitability risks beyond the inventory issue.

Still, the company's balance sheet position remains strong. The record free cash flow and expanded $3 billion buyback authorization provide financial flexibility, and management reaffirmed its long-term objective of generating more than $2.2 billion in free cash flow annually through fiscal 2028.

In a separate development, CooperCompanies concluded its strategic review of CooperSurgical and decided to retain the business. The board said offers received were not in shareholders' best interest, citing a temporary valuation disconnect due to competitive and litigation-related factors.

What Comes Next?

The inventory reduction cycle at CooperVision is the dominant near-term variable. Management expects most of the U.S. channel inventory correction to be completed by the end of fiscal 2026. If that timeline holds, the revenue trajectory could stabilize in early fiscal 2027.

Investors may also watch how the company deploys its expanded buyback authorization. With $339 million already repurchased in Q3, the added $1 billion in capacity gives management meaningful optionality if the stock remains near current levels.

Pre-market liquidity can be thinner than during regular trading, and volume confirmation during the open session will matter for the intraday trajectory. With full-year guidance now below consensus across both EPS and revenue, analysts will likely revise estimates downward, and the stock may face near-term pressure until management provides evidence that the inventory cycle is turning.
